It can re-crop the head to the preset's guide, resize the file and, where the rules allow it, put a plain background behind the subject. It cannot repair shadows, glare, a wrong expression or a pose problem, so take a new photo for those. Check the rejection notice for the exact reason first.

The face detector needs a clear, front-facing, well-lit photo. When it finds no face, the preview falls back to a centered crop instead of a face-positioned one. Choose a sharper photo with the whole face visible and no strong shadows, then run Preview crop again.

It depends on the DPI you choose. At 300 DPI a 2×2 inch photo is 600 × 600 pixels, and at 600 DPI it is 1200 × 1200. The tool does not enlarge a small photo; for printing it lowers the DPI instead so the result stays sharp. Check the portal's own pixel limits before you upload.
