No — rotation in pdf-lib changes the page's rotation property, not the page content. Text remains selectable, images stay at full resolution, and the file size is essentially unchanged. This is far better than rotating via a screenshot or by re-rendering through a print dialog.

PDF rotation is restricted to multiples of 90° by the PDF specification — 0°, 90°, 180°, 270°. For arbitrary angles you would need to rasterise the page (which we don't do here to preserve quality). Use a desktop editor for non-standard rotations.

"Apply to all" sets one rotation angle for every page at once — fastest when the whole document was scanned sideways. "Per page" lets you click through thumbnails and give each page its own angle, which is what you need when only some pages in a scan came out rotated wrong.

Use the 0° / 90° / 180° / 270° buttons in the Rotation panel to jump straight to the angle you want in one click, instead of pressing "Rotate all 90° CW" multiple times to reach 180° or 270°. In Per page mode these buttons apply only to whichever thumbnail is currently selected.

Start in "Apply to all" mode and click "Rotate all 90° CW" (or CCW) to fix the majority of pages in one action, then switch to "Per page" mode, click each problem thumbnail, and set its individual angle from the 0°/90°/180°/270° grid. Combining the bulk rotation with targeted per-page fixes handles a mixed-orientation scan far faster than rotating every page individually from the start.
